These two documents provide (in flow-diagram format) the following non-medical information/referral routes for GPs:
1. Low to medium dementia-related community needs:
· Information, 6-monthly check-in calls and dementia group support for people with dementia
· Dementia Advice Sheffield (Age UK Sheffield) - for onward referral to local People Keeping Well dementia support and groups (and Young Onset Dementia Hub for under 65s)

2. Higher dementia-related community needs:
· Information and support for up-to-6-weeks for people with dementia who are struggling to cope

3. Dementia Daytime Activities:
· Full-day engaging person-centred activities for the person with dementia
· A break for the carer
