Example letter regarding Hep B Contacts
Dear Doctor/Nurse
(Insert name of patient), a patient registered at Page Hall Medical Centre has given consent to contact you regarding their diagnosis of active Hepatitis B infection.
The patient has household contacts who are registered at your practice. These contacts are: (insert name(s)).
Guidance from the DOH Green Book states that all household contacts of patients with Hepatitis B should be tested and then if required, vaccinated against Hepatitis B.
We would be grateful if you would arrange for this to be done for your registered patients.
