Sheffield Mencap and Gateway are commissioned by NHS SY ICB to support practices, through offering individual tailored outreach and support to patients to help ensure uptake of their annual health checks and vaccinations.

The service also offers support and training from the project’s Learning Disability and Autism Physical Health Improvement Nurse.
