Two Hour Urgent Community Response ('2 hr UCR') - Service Offer
The NHS Long Term Plan is committed to bringing care and support to people in their own homes and equally to those who live in Care homes at times of a health and social care crisis. In Sheffield we have a well-established Urgent Community Response Team (Active Recovery) who aim to deliver support to individuals within 2 hours of referral as per NHSE guidelines.
The team have been working hard to iterate the offer to ensure patients receive what matters to them; and to provide an efficient service delivering excellence of care.
The referral source can typically be from general practice, NHS 111, A&E/same day emergency care, frailty assessment units, ambulance services, self-referral, carer referral or community-based health and social care (including care homes).
We would like to share with our colleagues across Sheffield our service offer to support you in identifying patients who may be suitable for an urgent community response. Within the service offer document, you will find further details about the service, eligibility and how to make a referral.
