Guidance Description:

Patients on the low income scheme should obtain a HC2 or HC3 certificate  and fill in a HC1 form which is a detailed assessment of their finances. We hold a stock of these in cashiers. The booklet giving details of help with health costs is a HC11 which is downloadable or readable on the Dept of Health website: https://www.nhsbsa.nhs.uk/nhs-help-health-costs

The attendance certificate referred to is a simple piece of paper held by the various hospital depts., supplied on request by the patient and it just confirms to us that the patient has attended a particular appointment.
