Sheffield Mind are commissioned by NHS SY ICB to support practices, through offering individual tailored health engagement outreach and support to patients living with severe mental illness (SMI) to help ensure uptake of their annual physical health checks, vaccinations, and national screening.
The service also offers a Cost of Living project for SMI patients, which can help encourage resilience-building through problem-solving and signpost to right resources to reduce the impact of financial worries.
