Description: The Cognition and Hearing Service is for patients who require significant adaptations to hearing assessment and

rehabilitation to be able to access these services.  This can be due to intellectual disability or other significant cognitive difficulties.  This form is only for patients requiring this specialist service.  The usual ERS referral process should be used for all other patients requiring a hearing assessment.
