This is the revised GP Feedback on Provider Interface Issues Form.  This form enables GPs, practice teams, and NHS colleagues to raise systemic GP–provider interface concerns to support learning and service improvement across South Yorkshire.  No patient identifiable information is to be submitted.  Any patient safety concerns should be reported directly to the provider of the service.  But examples can be provided to support the impact of the concern on the patient pathway.
