My Personal Wishes is a tool to help individuals record their wishes, feelings, beliefs, and values to guide future care and treatment. It has been developed by the South Yorkshire ICB Palliative Care Clinical Reference Group, with patient and family engagement.
As a professional, you can
Use this document to support conversations about advance care planning and ensure that the person’s preferences are understood and respected.
Share with an individual and their family to raise awareness of advance care planning, and for them to complete it in their own time.
Please encourage the individual to share this plan with family members and others involved in their care. Where possible, scan or upload the completed document to your case management system.
Ask the person to keep this document with their ReSPECT plan, if they have one and any other important papers, so it is readily available when needed.
Best Practice Tips for Professionals
Introduce the document early: Explain that it helps ensure care aligns with what matters most to the person.
Use open questions: Ask about values, preferences, and what is important to them in everyday life and future care.
Work at their pace: Allow time for reflection and reassure them that sections can be completed gradually.
Encourage sharing: Suggest involving family or trusted individuals in the discussion.
Review regularly: Check if wishes have changed and update the document as needed.
Respect confidentiality: Confirm who the person wants to share the plan with and who should not be informed.
